Sports physicals for 2014-2015 school year

Students participating on athletic teams must have a sport physical on file. The school has made arrangements with a registered nurse and two doctors to give these physicals at the school for a cost of $5.
The preliminary exam with height, weight, pulse and blood pressure will be done on Thursday, April 17, at 1:30-3:30 p.m. with Nurse Jessica Hinkle. Dr. Kapraly will be doing the girls’ physicals on Tuesday, April 22 from 8:30-10:30 a.m. and on Thursday, April 24 from 1:30-3:30 p.m. The date and time for the boys’ physicals will be announced later.
This is a minimal physical exam to meet OHSAA requirements. It should not be used in lieu of a thorough wellness physical by your family physician. Many problems could be missed during this examine. Intermittent check-ups and reviewing your child’s immunization records by your family doctor are strongly suggested.
Parents must have the forms filled out, signed, and returned to the school office by Tuesday,
April 15, with $5 before the physical exam can be given. (The money goes back into the sport’s program.) Sports physicals can be completed by your family physician. However, the doctor must fill out and sign the OHSAA physical form. Please make certain all information is complete and the last page is signed by both the student AND parent.
