Emergency Contact and Medical Authorization Form

DCS requires an Emergency Contact and Medical Authorization form be completed at the start of each school year. One form needs to be completed for each child. This form is used by the office staff in case of an emergency. Additionally, the athletic coaches are required to carry a copy of this form if your child participates in school sports. A copy of this form will be sent home with each student the first week of school or you can download it here. If you have any questions contact the school office.
