Fall Sports Physical Forms Due by Aug. 1

If your child is wanting to participate in a DCS sport, they are required to  complete a physical exam with a medical professional. The physician needs to complete the required OHSAA PREPARTICIPATION PHYSICAL EVALUATION 2017-2018 form. The OHSAA deadline for student-athletics playing fall sports is August 1. Please, return completed forms to the respecitve coach or to the front office.
